can anyone tell me how they put on an icon and a picture at the bottom of thier message.

Yes, it does come with the booklet. The blue(Limited Edition) version contains the regular version of the CD inside(contains 40 page booklet in slot) and the DVD.

I'm going to un-pin this, since most people have already bought it and/or know what the official covers look like by now.
